Q: What does PCMH mean?
A: A patient-centered medical home (PCMH) makes each patient the focus of their care plan. The patient’s chosen provider works with a care team to coordinate all aspects of the patient’s care including visits to specialists. This approach improves safety, quality of care, and communication and it empowers patients to understand that they have a say in their own treatment.
